Teeth Whitening, also known as "bleaching", is a common procedure in general dentistry but most especially in the field of aesthetic dentistry. Many people consider white teeth to be an attractive feature of a smile.



Generally, a child's deciduous teeth are whiter than the adult's teeth that follow. As a person ages, the adult teeth often become darker due to changes in the mineral structure of the tooth as the enamel becomes less porous. Teeth can get darker by many causes. The most common include genetics, aging, consumption of staining substances (smoking, coffee, tea, red wine, cola and etc.), tetracycline (antibiotic) staining, excessive fluoride and old fillings.

The procedure to bleach teeth uses oxidizing agents such as hydrogen peroxide or carbamide peroxide to lighten the shade of the tooth. The oxidizing agent penetrates the porosities in the rod-like crystal structure enamel and oxidizes interprismatic stain deposits; over a period of time, the dentin layer is also bleached. The effects of bleaching can last for 2-7 years (vary to each individual), but could be shortened by cigarette smoking and colored food and drinks consumption.

There are 2 main methods of Bleaching;

In-Office and At-Home Bleaching. In-Office Bleaching is performed by a dentist. The dentist applies a high concentration of oxidizing agent for 30-45 minutes. A special light may be used to enhance the action of the whitening agent. In order to reduce the risk of chemical burns to the soft tissues, most in-office bleaching procedures use a light-cured protective layer that is carefuly painted on the gums and papilla (the lips of the gum between the teeth).

At-Home Bleaching, patients will be given a pair of specific whitening trays which is custom-made for each individual. First of all, dentist will take an impression of your teeth for a custom-fit mouthpiece tray to exactly fit the patient's teeth. This customization will allow for maximum contact between the whitening gel and the teeth. Over-the-counter whitening kits, however, are for free size that do not completely conform to the shape of the teeth, sometimes leaving the tips of the teeth (near the gumline) unbleached. Patients then will be given the bleaching agents which are typically lower percentages of hydrogen peroxide than does in dental clinics. Under the dentist's advice, patients will drop bleaching gels into bleaching trays and wear during the night for several hours a day for approximately 1-2 weeks. Bleaching at-home is considered cheap, safe and most long-lasting but it normally takes longer time than in-office bleaching to see the results.

Thus, it is highly recommended that patients whiten their teeth by both methods; in-office bleaching to stimulate their whiteness of teeth, then prolong their white teeth with "take-home bleaching".
